Use this form to report a first-hand safety or serious service issue involving a provider. Reports go to Provider Companion moderation and are not automatically published.

Privacy: you may report without giving us your phone or email. If you provide contact information for follow-up, it is stored for administrators and is not displayed to the provider. Do not include unnecessary home addresses, financial account information, or intimate details.
